The Colony at Eden Prairie
Transitional Care Center specializes in rehabilitation, short-term
care programs required following surgery, injury or illness. These
include cardiac care, recovery from stroke, hip and knee
replacement, CHF, diabetic management and wound care.
Our
Transitional Care Center provides disabilities treatment so
residents can learn how to function and live within their maximum
physical and mental capabilities. For our residents' convenience,
rehabilitative therapies are administered in-house by Fairview
Rehab. The congenial rehab staff is highly-qualified and focused on
each resident's circumstances and needs.
